Job Description:

Summary: The Psychiatric Case Manager provides support to clients, clinicians and psychiatric staff, facilitates the authorization process as needed and helps to coordinate care among Health Center sites and in the community.

SPECIFIC DUTIES AND RESPONSIBILITIES: All duties are subject to accommodation in accordance with the Americans with Disabilities Act (ADA)

  • Work closely with Psychiatrist and Mental Health Therapists to assist with psychiatric patients.
  • Review charts prior to psychiatric visit to ensure all relevant material is accessible to Provider.
  • Process walk-in intakes, especially from Internal Medicine.
  • Under direction of Psychiatrist or Psychiatric APRN, prepare treatment plans for "medication management only" clients.
    • Provide support for crisis invention, as needed.
    • Interact with patients, families, agencies, and other disciplines with tact and confidentiality.
    • Help manage medication storage and inventory.
    • Other duties as assigned.

Experience and Skills:

Qualifications:

  • Experience working in a mental health environment, preferably acute care.
  • Knowledge of major psychiatric illnesses.
  • Excellent writing and verbal skills.
  • Computer literate.
  • Spanish speaking capability preferred.

Education and/or Experience: Bachelor’s Degree in Human Service Field

Specific vision abilities required by this job include close vision, depth perception and the ability to adjust focus. Generally, the working conditions are good with little or no exposure to extremes in health, safety hazards and/or hazardous materials. This person must have the ability to travel as required to work with staff.
