Job Description:

Summary: The Enhanced Care Coordinator works in collaboration and partnership with Medicaid enrolled patients coordinating their care with other providers, family members and community resources in a team approach and as a patient advocate. The ECC is a grant funded position through the Medicaid program. Goals of this program include but are not limited to, enhancing health and wellbeing, improving patient satisfaction, reducing Medicaid costs, and increasing utilization of primary/preventative care through enhanced care coordination.

SPECIFIC DUTIES AND RESPONSIBILITIES: All duties are subject to accommodation in accordance with the Americans with Disabilities Act (ADA)

  • Utilizes reports of patients with gaps in care who are at high risk for chronic illness to deliver outreach to patients who need medical appointments.
  • Maintains enhanced care management services with assigned case load of enrolled patients.
  • Participates in multidisciplinary and integrated care meetings related to quality improvement initiatives to review status for assigned caseload and clinical audits including, health outcomes, quality improvement activities, etc.
  • Assists with maintaining accurate and updated care coordination records, including electronic records for care plans, outreach activities, and correspondence with internal and external resources generating reporting analysis.
  • Ability to codify data, generate data reports, analyze reports and take corrective actions to improve desired program outcome.
  • Supervises PCMH+ Care coordinators.
  • Other duties as assigned.

Experience and Skills:

Qualifications:

  • Experience working in a fast paced medical facility, preferably internal medicine and/or pediatrics.
  • Demonstrated understanding of case management and care coordination.
  • Demonstrated knowledge of EHR systems preferably Next-Gen.
  • Excellent writing and verbal skills.
  • Bi-Lingual Spanish speaking preferred.
  • Minimum 3 years’ experience in supervision required

Education and/or Experience: Bachelor’s Degree required preferably in a medical care field. Minimum 3 years’ experience in a clinical setting required.
