Job Description:

Summary: Collaborates with providers and other multidisciplinary team members to ensure the effective and efficient delivery of comprehensive primary health care services to all patients.

SPECIFIC DUTIES AND RESPONSIBILITIES: All duties are subject to accommodation in accordance with the Americans with Disabilities Act (ADA)

  1. Demonstrates strong nursing knowledge base and skill in job performance according to established nursing policies and procedures.
  2. Protects patients and employees by adhering to infection-control policies and protocols, medication administration and storage procedures,
  3. Maintains patient confidence and protects operations by keeping information confidential.
  4. Maintains professional and technical knowledge by attending educational workshops; reviewing professional publications; and establishing personal networks;
  5. Ensures established quality assurance, CLIA waived tests, Joint Commission, Chronic Care model standards and are performed according to policy.
  6. Promotes patient's independence by establishing patient care goals; teaching patient, friends, and family to understand condition, medications, and self-care skills; answering questions.
  7. Adept at utilizing the electronic medical record to facilitate and support critical thinking and decision making, such as in the nursing process, and documenting the delivery of patient care.

 

Experience and Skills:

Qualifications:

  • Holds a current Connecticut Registered Nurse’s license
  • Provides effective and professional leadership in an outpatient setting serving a special and diverse population group.
  • Computer literate.
  • Education and/or Experience: Graduate of an accredited nursing program.
