Job Description:
Summary: Collaborates with providers and other
multidisciplinary team members to ensure the effective and efficient
delivery of comprehensive primary health care services to all patients.
SPECIFIC DUTIES AND RESPONSIBILITIES: All

- Demonstrates strong nursing knowledge base and skill in job
performance according to established nursing policies and procedures.
- Protects patients and employees by adhering to infection-control
policies and protocols, medication administration and storage procedures,
- Maintains patient confidence and protects operations by keeping
information confidential.
- Maintains professional and technical knowledge by attending
educational workshops; reviewing professional publications; and
establishing personal networks;
- Ensures established quality assurance, CLIA waived tests, Joint
Commission, Chronic Care model standards and are performed according
to policy.
- Promotes patient's independence by establishing patient care
goals; teaching patient, friends, and family to understand
condition, medications, and self-care skills; answering questions.
- Adept at utilizing the electronic medical record to facilitate and
support critical thinking and decision making, such as in the
nursing process, and documenting the delivery of patient care.
