Job Description:

Summary: Provides assessment, counseling and therapy to individuals, couples, families and groups in order to prevent or interrupt the perpetuation of dysfunctional intergenerational patterns such as substance abuse and family violence, and to promote life enhancing coping skills.

SPECIFIC DUTIES AND RESPONSIBILITIES: All duties are subject to accommodation in accordance with the Americans with Disabilities Act (ADA)

  • Provides psychosocial assessment, intervention, treatment and referral as appropriate to patients in need of these services, including children, adolescents and adults.
  • Facilitates group therapy sessions as assigned.
  • Consults with psychiatrist, other department members and medical staff regarding patient care.
  • Advocates, as appropriate, with other systems, such as schools and DCF.
  • Obtains pre-authorizations and re-authorizations from insurance companies to ensure reimbursement for services rendered.
  • Maintains client records and provides other documentation as required.
  • Performs all other duties as assigned.

Experience and Skills:

Qualifications:

  • Good organizational and communication skills.
  • Computer literate including Microsoft Office.
  • Ability to work well with culturally diverse populations.

Education and/or Experience:

Graduate degree in Social Work, Counseling or Human Services. CT License preferred. Experience in providing services to medically underserved and culturally diverse populations. Ability to utilize Electronic Health Record (EHR)

Specific vision abilities required by this job include close vision, depth perception and the ability to adjust focus. Generally, the working conditions are good with little or no exposure to extremes in health, safety hazards and/or hazardous materials. This person must have the ability to travel as required to work with staff.
